Our Community Support and Health Home Teams continue to grow and so we are looking for Adult Case Managers to provide case management services to individuals with severe and persistent mental illness and substance abuse. Our goal is to help people live as independently and as fully integrated within the community as possible. As case managers you will provide supportive assistance to psychiatrically disabled adults and their families in order to maintain and improve their level of functioning in the community. In this role you will assist in identifying clients’ needs, goals and their strengths. Case management services provided to clients include; assess and evaluate all clients on an ongoing basis, provide support services and/or crisis intervention as needed. Develop, design and implement treatment plan goals with clients to promote their recovery. This also includes assisting clients with community resources to support their mental health and substance abuse needs. Candidates must demonstrate strong writing and communication skills, ability to multitask and have excellent organizational skills.

Education/Licensing Requirements: AA in a human services field required. Approved driver's record and reliable transportation required! Knowledge of substance abuse is a plus. Bilingual candidates are encouraged to apply.
