Residential Case Manager- Singleton

A full-time Residential Case Manager is needed for our group home in Pascoag. The case manager will assist in the delivery of psychiatric, rehabilitation, and supportive services to adults living in this setting. Provide case management services in accordance with the treatment plans, goals, philosophy, and standards defined by supervisor and Community Care Alliance. Work with and support our clients to achieve their highest potential through continuous assessment, monitoring, evaluation, and intervention when needed. Case Management services include: medication monitoring and supervision, crisis management and support, behavior planning and modification, and family and social support involvement in treatment. Educate clients, their families and significant others, re: wellness and health prevention activities, mental health issues, substance abuse and the impact of one upon the other. Candidates must be able to work well within a team!
Education/Licensing Requirements: Associates Degree in Human Services is a requirement; BA/BS is preferred. If you have the Associates and experience then apply online now! Must have current CPR/AED & First Aid or willing to obtain.
